Patient Forms

If you are a new patient to our office, please call our office to request a Patient Forms Packet.  These forms can be completed online or printed. By having this completed information sent to us prior to your appointment, it will allow us to provide more efficient service at your initial visit.  Thank you and please contact our office if you have any questions.

Payment Options

For your convenience, we accept cash, personal checks, money orders, and most major credit cards. Payment is expected at the time services are performed.  Feel free to contact our office If your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.
